RESUMO OBJETIVO: Analisar a relação entre condições de promoção de saúde nas escolas e o consumo de álcool e outras drogas pelos escolares. MÉTODOS: Estudo transversal de base escolar, com amostra probabilística de 3.464 escolares de 12 a 17 anos de todas as escolas de Lajeado e Sapiranga, RS, e 53 gestores das mesmas escolas; dados coletados em 2012. Os relatos do uso de 2012 de tabaco, álcool e drogas ilícitas foram tomados como desfechos e o escore de promoção de saúde no ambiente das escolas, como exposição de interesse. Os dados foram submetidos à análise multinível. RESULTADOS: A prevalência do uso anual de tabaco foi 9,8% (IC95% 8,8-10,8), álcool 46,2% (IC95% 44,5-47,8) e outras drogas 10,9% (IC95% 9,9-12,0). Na análise bruta, apenas o uso de tabaco esteve associado a escolas menos promotoras (OR = 1,89; IC95% 1,16-3,09) quando comparado as com melhores condições. Essa associação perdeu significância estatística na análise ajustada (OR = 1,27; IC95% 0,74-2,19). CONCLUSÕES: Os efeitos do ambiente escolar quanto ao uso de drogas, especialmente tabaco e álcool, manifestam-se principalmente pelas condições individuais e familiares dos adolescentes.

In the crude analysis, only the use of tobacco was associated with less health promoting schools (OR = 1.89, 95%CI 1.16-3.09) when compared to those with better conditions. This association lost statistical significance in the adjusted analysis (OR = 1.27, 95%CI 0.74-2.19). CONCLUSIONS: The effects of the school environment on the use of drugs, especially tobacco and alcohol, are manifested mainly by the individual and family conditions of the adolescents.

Foi proposto o Escore de Promoção de Saúde no Ambiente Escolar (EPSAE) e foi estimada a distribuição dos escolares segundo esse escore e segundo odds ratio (OR) brutas e ajustadas, por regressão ordinal, para exposição dos escolares a escolas com escores mais elevados, segundo as variáveis independentes. RESULTADOS Um escolar tem mais probabilidade de frequentar escola com EPSAE elevado na região Sul (OR = 2,80; IC95% 2,67–2,93) se a escola for privada privada (OR = 4,52; IC95% 4,25–4,81) e estiver localizada em capital de estado e se o escolar tiver 15 anos de idade ou mais, exercer atividade remunerada ou tiver mãe ou pai com escolaridade superior. CONCLUSÕES São importantes as desigualdades entre regiões do país e entre redes de ensino, tornando clara a necessidade de alocação de recursos e ações que promovam maior equidade.

O objetivo desta pesquisa foi analisar a relação entre o padrão de consumo de crack nos últimos seis meses de uso ativo e a condição de abstinência ou não no momento das entrevistas. Trata-se de um estudo transversal com amostragem de conveniência, sendo que foram entrevistadas 495 pessoas entre os 14 e os 54 anos de idade. Foram estimadas razões de prevalência por Regressão de Poisson robusta para a condição abstinente por 12 semanas ou mais, segundo os padrões de consumo referidos, ajustando para sexo, idade, escolaridade, tempo desde o primeiro contato com a droga, uso de medicação e hospitalização em função do crack. Identificou-se associação entre o uso frequente e pesado e a cessação do consumo (RP 1,06 [IC95%: 1,01 - 1,12] p = 0,019). Esse achado amplia o leque de particularidades em relação ao crack e reforça os investimentos terapêuticos para todos os padrões de consumo.

This study aimed to analyze the relationship between the patterns of crack cocaine use among active users over the past six months and abstinence (or not) at the time of the interviews. A cross-sectional study was conducted in a convenience sample of 495 crack users aged 14 to 54 years. Prevalence ratios of the variable abstinent for at least 12 weeks were estimated using Poisson regression with robust variance according to the patterns of use, adjusting for sex, age, education, time since first contact trying crack cocaine, and treatments and hospitalization associated with crack use. There was an association between regular and heavy use and cessation of drug use (PR 1.06 [95% CI: 1.01 to 1.12] p = 0.019). This finding expands the range of particularities regarding the use of crack and reinforces the need for investments in crack cocaine addiction treatments for all use patterns.

RESUMO OBJETIVO Validar instrumento de avaliação da promoção de saúde no ambiente escolar. MÉTODOS Para validação do instrumento de pesquisa, foi elaborado questionário com base em diretrizes da Organização Mundial da Saúde, mas adequado ao contexto de saúde escolar no Brasil, com 60 itens, sendo 40 dirigidos aos gestores e 20 para observação direta pelo entrevistador. A validação de conteúdo dos itens foi feita pela técnica Delphi, com aplicação do instrumento em 53 escolas de dois municípios, de porte médio, da região Sul do País. Foram realizadas análises de confiabilidade (alpha de Cronbach e split-half) e de validade (análise de componentes principais). RESULTADOS O instrumento permaneceu composto por 28 itens, distribuídos em três dimensões: pedagógica, estrutural e relacional. Os componentes resultantes apresentaram boas cargas fatoriais (> 0,4) e confiabilidade aceitável (> 0,6) para a maioria dos itens. A dimensão pedagógica identifica atividades educativas que abordem drogas e sexualidade, violência e preconceitos, autocuidados e paz e qualidade de vida. Estrutura se divide em acesso, estrutura sanitária e conservação e equipamentos. A dimensão relacional inclui relações na escola e relações com a comunidade. CONCLUSÕES O instrumento proposto apresenta valores satisfatórios de validade e confiabilidade, englobando aspectos relevantes à promoção de saúde nas escolas. Seu emprego permite retratar condições de promoção de saúde às quais estão expostos os estudantes vinculados a cada instituição de ensino. Como possui itens de observação direta pelo pesquisador, deve ser usado apenas em períodos de atividade plena e regular da escola.

METHODS A questionnaire, based on guidelines from the World Health Organization and in line with the Brazilian school health context, was developed to validate the research instrument. There were 60 items in the instrument that included 40 questions for the school manager and 20 items with direct observations made by the interviewer. The items’ content validation was performed using the Delphi technique, with the instrument being applied in 53 schools from two medium-sized cities in the South region of Brazil. Reliability (Cronbach’s alpha and split-half) and validity (principal component analysis) analyses were performed. RESULTS The final instrument remained composed of 28 items, distributed into three dimensions: pedagogical, structural and relational. The resulting components showed good factorial loads (> 0.4) and acceptable reliability (> 0.6) for most items. The pedagogical dimension identifies educational activities regarding drugs and sexuality, violence and prejudice, auto care and peace and quality of life. The structural dimension is comprised of access, sanitary structure, and conservation and equipment. The relational dimension includes relationships within the school and with the community. CONCLUSIONS The proposed instrument presents satisfactory validity and reliability values, which include aspects relevant to promote health in schools. Its use allows the description of the health promotion conditions to which students from each educational institution are exposed. Because this instrument includes items directly observed by the investigator, it should only be used during periods when there are full and regular activities at the school in question.

Contamination of enteral diets represents a high risk of compromising the patient's medical condition. To assess the microbiological quality and aseptic conditions in the preparation and administration of handmade and industrialized enteral diets offered in a hospital in the Valley of Jequitinhonha, MG, Brazil, we performed a microbiological analysis of 50 samples of diets and 27 samples of surfaces, utensils, and water used in the preparation of the diets. In addition, we assessed the good handling practices of enteral diets according to the requirements specified by the Brazilian legislation. Both kinds of enteral diets showed contamination by coliforms and Pseudomonas spp. No sample was positive for Staphylococcus aureus and Salmonella spp. On the other hand, Listeria spp. was detected in only one sample of handmade diets. Contamination was significantly higher in the handmade preparations (p < 0.05). Nonconformities were detected with respect to good handling practices, which may compromise the diet safety. The results indicate that the sanitary quality of the enteral diets is unsatisfactory, especially handmade diets. Contamination by Pseudomonas spp. is significant because it is often involved in infection episodes. With regard to aseptic practices, it was observed the need of implementing new procedures for handling enteral diets.
